In SWAT 3 there is the report command "down" for wounded enemies and "neutralized" for dead enemies.

However, it seems the game sometimes mixes these two up.
Sometimes an enemy is apparently dead, but when I try to report it, it only lets me choose "down".

Also, if a person is down or neutralized, do you still have to call for evacuation?

When a member of your squad shoots a suspect, he will automatically report the status of the suspect. If the suspect is KIA, your squad member will report "Suspect neutralized". If the suspect is still alive, they will report "Suspect Down". In these cases, the option to report the correct status of the suspect will be grayed out, and only the other status (the wrong one) will be selectable (if you report the wrong status, your team will correct you in the most embarrassing way possible). Since your squad already reported the status for you, there is no action required (only the evac for the cases where the suspect is down).

When the suspect has been shot by you, then you'll need to correctly report his condition (wait to see if he moves/blinks to see if he's alive). Then, if he's still alive, you'll need to call for evacuation. Don't worry about evacuating dead bodies.

I'd been wondering what those grayed out status options were all about, and then often picked the only one that was left (the wrong one).

Bit of a dickish move for the game to offer you the wrong one ;-)
The first time my team member corrected me, I said "Don't contradict me in front of the team!" Then I shot him. Then I died. Being SWAT sure is hard!
