I had this problem with the previous version of this game. Figured it might be fixed with v5.0.3. Still having the same problem. I can change video and audio setting and watch the credits but can't actually start a game because "Load game" and "New game" options are greyed out.

I tried contacting GOG support but the idiot chatbot refuses to create a ticket and there doesn't seem to any other way to contact GOG for tech support. What a stupid support set up.

I downloaded the installers and do not have GOG Galaxy installed. Also made sure to allow Starsand through my firewall in case it needed online access, which I think is very questionable if it does for a drm free game, but doesn't make a difference.

Wondering if GOG Galaxy installations are the only way to play the game.

About to ask for a refund. Very frustrated.

i refunded the game
Problem still exists: Tested 0.5.3 and 0.5.4

=> No GOG Galaxy installed
=> Running Windows as User (running as Admin didn't fix the issue)
=> It might be just a missing save game folder... though I do not know where to look for it

Couldn't find any log or error messages.

"New Game" button greyed out...
The disabled NEW GAME button issue is still present on version 0.5.6.
I don't (and don't want to) use GOG GALAXY at all... So I don't know if that version works fine.
I will wait a little but I've bought this game following the great expectations from what I've seen in the videos on internet and I'm very disappointed because I cannot play it.

I've given admin permissions to the game, so the issue is not for that reason.

I figured it out - you have to install GOG Galaxy and login to it. Good news - you don't need to redownload a game, just run GOG Galaxy, login and minimalise GOG Galaxy window. Then start a game installed from offline installer.
wait 10 seconds for it to white out to use.
Please have a look in the log files:

%USERPROFILE%\AppData\LocalLow\Tunnel Vision Studio\Starsand

I got an error which says that no "GogSaves" folder exists.
My log says:
Fallback handler could not load library C:/GOG Games/Starsand/Starsand_Data/Mono/EOSSDK-Win64-Shipping

Which means the devs need to fix how/where they look for their files.
I had to reinstall when this happened to me. After which the New Game option became active a few moments after the game finished loading.
For this error, go to %USERPROFILE%\AppData\LocalLow\Tunnel Vision Studio\Starsand and make a new folder named GogSaves

Re-launch the game and the new game option will be available.
