Fixed it.
I'm running on Windows 11.
I ran it from the default GOG directory (D:\GOG Galaxy\Games\Star Trek Bridge Commander in my case). I didn't have to install to Program Files or wherever.
First you need to generate the options.cfg file which you do by running the game once.
If you have problems running the game for the first time, consider setting the Compatibility Mode on "Launch Star Trek™ - Bridge Commander" short cut, or "stbc.exe" to "Run this program in compatibility mode for: Windows XP (Service Pack2)".
You can also set compatibility to "Run this program as administator."
If you still have problems running the game for the first time, try setting "Disable full-screen optimisations" in the Compatibility Mode tab.
You only need to set the Compatibility mode the first time you run it, if it doesn't run as is. You can then revert the Compatibility Mode by unclicking everything after the game has run for the first time.
Once you manage to run the game (just going to the Settings menu after ESCaping from the intro screens should be sufficient) you'll have to CTRL+ALT+DELETE -> Sign Out to get out of the game.
When you log back in to you PC, open File Explorer and go to your "Star Trek Bridge Commander" directory.
Backup the "options.cfg" file in case something breaks and you have to restore it. I just copy it to a file called "options.cfg.orig". Name doesn't matter, as long it's different from the original name and you can find it easily if you need it.
Then, edit the actual "options.cfg" file. I open it in Sublime Text as there are some special binary characters in the file and Sublime Text has no problem with them. You can actually delete those special characters. Sublime Text shows them as "<0x00>"
I don't recommend opening in Word, or Wordpad as they tend to put in special characters. Notepad should work fine. I think it just drops the binary characters, but that doesn't harm the game.
You only need to turn off Full Screen mode to get the game to play with sound.
Just change "Fullscreen Mode|1" to "Fullscreen Mode|0".
The problem I have is that I have a very wide screen and the window opens in the top left of my monitor. There's no title bar to drag it to the centre of the screen.
