I'm having the same problem with the letters not showing up. I've tried all the solutions and none of the work. Anyone got any further with this?
EDIT: Also tried cheating and just entering the letters in order. This doesn't work, as the last letter refuses to be clicked.

Blinky802: I had the same problem and ended up starting the game with Sntrm.exe and not using dxwnd and I was able to complete the puzzle. In fact it seemed to clear up many of my other crashing problems.

Johnny_Bright: Thanks, Blinky, this time I was about to give up playing Sanitarium (before getting insane), but your solution worked for me too. The funny part is, the game is so good I keep looking for solutions for the bugs so I can continue playing it. I wonder if it was underrated at its time, because it never got a sequence.

I also really enjoyed the game, it just came out at the tail end of what I call the adventure game decline - publishers just stopped making adventure games as they were getting vastly outsold by the shooters, RPGs and strategy games.
Yes this happened to me with the original. Damn, hoped it had been fixed. I also had an issue with character walking backwards (pumpkin patch, entire final chapter).
The solution? VirtualPC running windows 98. It's literally the only solution. I cheated my way by "assuming" where my cursor was and moving slowly to catch the letters. It worked, but not so many folks have that kind of patience. If played under virtualPC it works fine.

If you can fix this gog - I will love you long time. Sanitarium is buggy but amazing

iGaboru: I was getting the same annoying flickering and read on another thread that activating Win 98 compatibility will fix it, and so it did. If you don't know how to do it, right-click on the Sanitarium shortcut and select Properties. Then click on the "Compatibility" tab and select Windows 98 / Me.

When i'm doing this,i'm losing my save game.
Nevermind, it looks like i was dumb enough not to look at the support articles
