The culprit was anti-aliasing. The default NVIDIA setting is enabled at x4. Didn't realize that until now. Turning it off got rid of it. Anisotropic filtering had no effect.

Also, downloading the latest version of dgVoodoo (2.77 as of today) fixed the problem with cinematics not showing up. Now there is not a single reason not to use it to make the game look as crisp as if you were running the highest native resolution your display can support. 1920x1440 looks amazing on my 2560x1440 display.
