Say you have 4 equipment slots (the circles on the left) and each has an item with +1 for a CA, would each items +1 stack or is it only the slot (circle) that is in "use" that counts?

If you're referring to the quickslots on the bottom of the main screen, then only the active equipment's bonus is applied. The rest isn't equipped, but only easily accessible.
