When I'm tabbed into another window (while reading the PDF manual, for example), frequently when I tab back to the DOSBox window again, the game crashes. The only option I have then is to restart the game/DOSBox from scratch.

So, being familiar with DOSBox settings from years of using it, I figured setting the priority in the dosboxrealms1.conf to "pause" whenever the window isn't focused should fix the problem. But I can't, because that entire folder is set to read-only and it's very difficult to change this in Windows 7.

Any ideas? If I can't fix this, the game is basically unplayable for me. I have to be able to refer to the manual frequently, this is necessary for character generation.

Easiest way would be either to disable User Account Control or to reinstall somewhere outside Program Files to prevent future problems.
Alternatively you should be able to edit .conf file be either opening it as administrator or simply copying it elsewhere, edit, and copying back.

Yeah, eveyrtime I have to look up pass codes in the beginning of the game I have to switch to the passcodes pdf. Then I switch back to the game and screen stays small and it's not possible for me to generate any character. I can only type in the name of my character and that's all...someone has any ideas how to fix this?

Ever thought about printing a hard copy of the passcodes?

Cheap tablet with a PDF reader works wonders for these situations, heh.
