With today's Update 1 new cvar was introduced to turn off Damage Indicators (not to be confused with Hit Markers), those red line pointers indicating directions for incoming attacks.
Devs didn't have time to implement specific option in the menu, so perhaps it will be added in the next update.
In order to disable those Damage Indicators you need to open the console and paste the following text and hit enter:
scr_damageindicators 0
Make sure that your GOG version of Quake II is updated to recent version (Quake II 1.0.5967), it could take some time.
