Installed Planescape Torment, installed the widescreen and UI mods, everything works flawlessly, the game looks really good except... When there is dialogue, I can't hear it. I get the very first line spoken when you first start out in the game and then nothing after that.

I have a Soundmax integrated sound card so It's not compatible with creatives 'Alchemy' thing as far as I know. Does anyone know of a fix? I would really like to hear the dialogue as I play the game. Also, my drivers are up to date so it's not an issue with that.

There are no voices in the dialogue. Only the small snippet lines like the one you pointed out.

There are a few spoken lines but most of the text isn't. PS:T has some 400+ pages of dialogue and not only would it have been very expensive to record all those lines, there were also technological barriers. PCs of that time were nowhere near as powerful as today's machines and didn't have the performance needed to play mp3 sound while also running the game itself. Not using mp3 compression would have had some serious disc capacity issues and as it is, the game was originally released on 4 CDs. So it just couldn't be done at that time.
The previous two posters are correct: only some lines in the audio are voiced. At the time the game was released (1999), it would have been impossible to completely voice-over the entire thing.

I know, this is disappointing to many people who first play games such as BG, BG2, and PS:T, more than 10 years after their release :) (The much smaller Icewind Dale, from 2000, is fully voiced over, but it's much less epic and complex compared to Torment.)

Treat Planescape: Torment as a *very good* interactive book with clarifying illustrations, and you will be fine. I'm sure you'll love the story and will yet remember it in 2030... and you'll still be asking yourself why you didn't play it earlier :)

Just started this game and can't pick anything up. I'm looking for a scalpel and click on the tables but no scalpel. Also, the audio just keeps looping the same 2 1/2 words over and over unless I open the inventory screen, or save/load screen, etc.

Running Windows 7 Home Premium 64 bit
I haven't played the game in what seems like ages, but if I recall the scalpel is not on a table but on one of the shelves / cupboard things, or something.

Aside from that, I don't think you actually *need* to take the scalpel. I think you can clobber the zombie with your fists if you really want to.
