I use WinRar to extract and install patches and mods myself, though there are other methods such as using the default Windows unzip and extract system. Install WinRar (if you don't already have it), then find the patch or mod you downloaded and double click it, it should bring up the contents of the zipped file in WinRar. Next, click "Extract to" at the top of the WinRar toolbar/menu then it should bring up another menu, navigate to the Planescape Torment folder and click on it once, the path info should read something like "C:\GOG Games\Planescape Torment" then click OK to extract.
Next, go into your Planescape Torment folder and find the exe files for each mod (should say something like "setup-ghostdog.exe" or something for example) and run them, a window with a black backgound and white text should come up and give you options for installing the mod, just read what it says and follow the instructions to install.
Hopefully I explained it right as i'm not a very good instructor. If you need further help i'll try and explain it better.

Note: You shouldn't have to use the WinRar method with the widescreen mod, but if you want to change resolution again you'll have to find the widescreen exe in the Planescape files and run it again.
