I've been playing for a few hours now and earned several levels, but when I click the level up button I am not given the option to assign weapon proficiencies. According to the character sheet for the Nameless One, I have 4 unused slots, 1 in fists and 1 in edged weapons. Am I doing something wrong?

You have to find actual trainers in this game to assign those points; each prof. has its' own trainer. I can't recall where they are, except scattered around Sigil - it's been too long since I played the game. Just explore and talk.

There's one in the north-western Hive (Porphilon?) and another in the Lower Ward (I think, might be the Upper Ward.. it's a Harmonium guy at the western entrance).

If you got weapon proficiencies as a fighter, and you switch to mage, do you still have those proficiencies as a mage?
SPOILERS!!!!!!

There are ONLY two trainers in weapon proficiency, so you do not want to kill everybody in sigth, heh.

If you want train with them you MUST be a FIGHTER.

First, in the Hive, Southern-West (was long ago since I played, next to this place where is the house, where you can sleep for free, after successfully accomplished quest for throwing out this psycho whom looking for his fork), called Profirion. You can not train with him without completing a quest - get his religious beads. He will train you only to the second level weapon proficiency, if you got the points.

Second, in The Lower Ward (this location with Siege Tower) is Korourn (or something like that, near west entrance, looks like warrior). He will train you for free, but you STILL MUST be a FIGHTER and got the points. You can train with him to fifth level of weapon proficiency, but you must attain 7 and 12 level FIGHTER to do that (on this level you get class FIGHTER specialization). Also you must already have two points of prociency in selected weapon (like trained with Profirion).

Third, yes YOU STILL GOT THOSE PROFICIENCES when you switch to other class.
