Not much mods really for this game, but here is a mod you should install to make it look and run better on modern systems:

thanks very helpful! :)
They should really think about adding this as an official Goodie, it's so big and looks like a pain to download from a DDL site.
They don't add and can't add unofficial things.

It's so big because it includes high res textures.
I wish they could just release the widescreen patch. i don't need highres textures or the enb series mod(i know that all features can be turned off...but still).

I cannot download from 4shared because i have to register(which i won't do). The file is not available at Dropbox.

They did a very nice job though!
It's also available at this moddb site, which should give you better access to mirrors etc.

Thank you for the Link!
Hey all, I just had some issues getting that widescreen patch working, and I thought I'd share in case they help anyone else. When I installed it and tried to run it, every time I would get "error starting renderer" (or words close to that) with a crash.

What I finally decided is that the mod installer makes some assumptions about your hardware that aren't necessarily true. (In my case, I think it was because it thought my hardware supported anti-aliasing within the game, but the game disagreed.) So If you're getting an error about the renderer, here's my generalized guide:

1)Re-Run the mod installer as normal. Pick whatever options are appropriate.

2)BEFORE running the game or doing anything else, go to <installdir>\data\ and look for CONFIG.INI. Back up this file in some fashion. Then run it with the GAME.EXE file.

3)After the game crashes with that error message, it'll drop to the game configurator. Go ahead and select the best options it allows you to select. (Like, in my case, antialiasing was not even an option.) Save, make sure it *launches*. You'll likely get corrupted graphics on launch. Don't worry about it. If the game launches at all, you're fine, quit out of it.

4)Re-open that CONFIG.INI. It's now going to have the settings that "worked." Compare those to the earlier file you backed up to see what's different. In my case, all I had to do is change the X and Y settings in the new Config file, keeping the other default settings, and the high-res patch worked fine.

However, you CAN twiddle with it further, like changing the DODrawDist parameter to give yourself a longer view distance.

Also, as a side note: If you've never dealt with the ENB Series mod, it uses a **LOT** of horsepower. If you install this patch (or anything else that uses ENB, ever) step one in troubleshooting poor FPS is to just shut it off. You'll usually see a 2x+ jump in your frame rate, even if ENB wasn't actually doing much.
For me the widescreen patch worked fine without any errors and the game runs smooth.
I have Win7 64bit, Intel Core i7 2.93 GHz, 4 GB RAM and Zotac GeForce GTX 460.
For those who would like to experience a yet crueler game, there is the Pathologic ambiguity mod. I cannot yet post links, but it is readily available in the English section of Ice Pick Lodge's forums.
Much as I adore Pathologic in its original state, I believe that this mod brings it closer to the core concept. Pathologic is an incredibly intense game, but at times it simply becomes a mechanical stat management experience. I am of the firm belief that in a true role-playing video game, or any video game with stats, really, the stats and variables should remain hidden from the player. The ambiguity mod does exactly this, more or less - it removes the initial sensory distortion from infections and causes them to be more or less invisible until they reach 25%, reduces the opacity of the player's marker on the map, and removes the message alerting the player of their success or failure at 0:00. This makes for a much more tense experience. Highly recommended for the crazy folk among us who found Pathologic a little too easy.
