Amiri has cleave and cleaving finish. Very rarely do either of these fire. Whenever I have to move her into melee range of two or more enemies, I try using Cleave. In spite of massive reach via oversized weapon, enlarge person, and now legendary proportions, nothing happens after the first hit. As for Cleaving Finish, it seems to fire, on very rare occasions, but usually it doesn't. She hits someone, they die, and the guy standing directly next to that is left alone. What am I missing?

From my non-expert understanding, all the extra Cleave feat attacks only cause damage if the original attack roll is successful, AND the attack roll against each additional enemy succeeds. The in-game combat log shows the results for every roll in every battle, so maybe look at that.
That could be it. It might be that the other attacks are sometimes missing. Easy to do in Pathfinder unless you debuff the enemy like crazy :)

It doesn't help that Amiri comes pre-equipped with a debuff to her AB by default, which 95% of players seem to ignore/are unaware of.

Getting bonuses to hit is so easy in this game if you know what you're doing, even at lower levels; eventually you're rarely going to miss at all.
