I've gotten a few 'do 100 of X' achievements. Without doing more then 1-2 at a time. For instance, I have the 'solve 100 kingdom problems' achievement when I've literally just gotten to the point of having my kingdom.

Dunno if I'm the only one who's reported this, but I hope I'm not.

In patch 1.02 apparently. I got the same bug at launch but I can't confirm if it's fixed because I stopped playing the game.

Same here. I've got the survivor achievement. While I haven't been keeping track of how many encounters I've had, I'm pretty confident that it's nowhere near 100.
