I know this has been posted in one form or another, but I cannot get a resolution. Now, I am experiencing the "slow motion" effect in this game, regardless of my video settings. I have a computer that is 7 years old at home that can run this game flawlessly. My computer here is only 2 years old and it is slowing down, so it is not a question of hardware capability. I have tried processor affinity, Vsync and nearly everything else out there to no avail. This is truly a frustrating problem, I cannot imagine how such a problem can exist without some solution being available on the internet (believe me, I have looked).
If you have any idea about the cause of this problem, please let me know.

I'll add that the game runs at a steady 150+ fps but it is still in slow motion.
Could you please post your system specs and OS anyway just so we have more of a complete picture of what at play here?
It is a laptop.
Intel Core 2 Duo T7500 2.2 GHz
2 GB RAM
ATI Mobility Radeon HD2300 w/ 128 MB dedicated video RAM (I think I have an additional 512MB of my system memory allocated for video use as well)
Windows Vista
Is the game running in slow motion as soon as you enter the level or does it take some time to get to slow motion?
As its a laptop make sure its well ventilated so its not overheating. Next check your power options, make sure its running in the desktop powersetting and not the energy saving settings. Painkiller is still a bit of a resource hog despite its age.
You could also check your page file with 2gb ram and a 128mb card you probably want 3.5-4gb of pagefile.
I also don't know if you run anything like cool and quiet, if so it may be a good idea to disable it while playing painkiller.
If you need any instructions on how to check or change any of this feel free to ask.
Lastly it sounds like you probably have done this already but if you haven't try disabling bloom and dynamic lighting in the games options.

It starts running in slow motion about 3 seconds into the first level, when the door slides closed in front of me at the cemetery.
My power options are good, no cool and quiet or anything, and I just changed the pagefile size. Now, a new phenomena I noticed: it sounds like my hard disk is pulling double duty and when every I hear the head advance, I'll get a short boost in speed. This leads me to believe that for some reason or another, the game isn't utilizing my system's memory properly.

How often do you defrag your harddrive? If it has been a while you could do that. It sounds like its taking longer then normal to read the game into your memory.

I defrag quite often; once or twice a month?
Let me add that the same problem has been happening with Blitzkrieg on my system. Which is strange because there is so little in common between the two. However, their differences may help isolate the problem.
If you haven't done so already contact gog support about the game. Include a dxdiag with your report as well to help them isolate the problem.
Nothing immediatly springs to mind at the moment and defragging once or twice a month is a good amount. I would still say to defrag it but only if you haven't done so since installing the game.
Have you managed to play anything more graphically intense then blitzkreig and painkiller on that laptop without the slowdown?
Try to determine the memory usage. One (simple) way would be to:
* Open Task Manager (Ctrl+Shift+Esc)
* Check the current physical memory usage under the Performance tab
* Start Painkiller
* Alt-Tab to Windows
* Check the physical memory usage again
If it's anywhere near the Total Physical Memory, it's going to swap ... which results in bad performance.

I think I see how this works. Essentially, programs like Blitzkrieg and Painkiller take a clock reading at the beginning and then base their game speed off that for the whole game.
I can run games like Left 4 Dead and Half-Life 2 fairly well.
I've given it a shot. It works pretty darn good. You just have to make sure you open 2 instances and set different affinity for each.
This is a pretty good work around.
I wish I could find a means of disabling Speedstep on my computer as I believe this is a main cause of my troubles.

My laptop is more than powerful enough to rip this thing to shreds, and it still had severe slowdown. That process workaround did the trick though!

Unfortunately, this also causes your processor to start running pretty hot, so it is really only a workaround.
