I downloaded Outcast today, installed it on my Windows 8.1 laptop and had the same problems. OC3.EXE "stopped working". By going into Properties and setting compatibility to Windows 98/ME and Run as Administrator (re: START.EXE, LOADER.EXE, OC1.EXE, OC2.EXE and OC3.EXE), the program actually runs just fine. I have an Intel HD 4000 and NVIDIA GT635M. No alterations to the graphics card settings were made.
