In the first town of the game came across some crates. Thought maybe you could break them and find things, so I used the smash command... and took damage. Curious now, I kept using it and died. Who thought this was a good idea to put in the game?

At least you die by bodyslamming a (theoretically strongly built) crate. In Ultima Underworld, you can take damage by jumping towards a floor that is sloping upward. I like to say that it is possible to die of a stubbed toe in that game. :D

I love me some classic game quirks, but some just make go... huh?

Your example is fantastic.
Speaking of game quirks, I think I once found a way, in MM5, to get a party where everyone was unconscious/dead/petrified/eradicated, but I could still make a move before the game decided that my party had wiped. I remember doing this in the 3rd town in Darkside, and it may have involved dismissing a party member.

By the way, AC is useless in the first town of MM4, as well as in many other areas of MM4 and MM5.

Also, this reminds me of some of the many unique deaths you can get in Nethack, including things like falling off your horse, or throwing items while levitating, or even falling from cloud nine.

One thing is, in Nethack, if something can damage you, it is possible for it to kill you, and you can get a unique death message that way.

*polymorphs into a dragon, genocides all humans, then quits*
