Just thought I'd make a post compiling my own version of some of these patches for MDK that I've found and compiled.
DOS patch
In the current Windows release of MDK, the DOS version isn't included at all, so here is a copy of the DOS version with all the necessary files, with the CD check removed. Just extract to your MDK installation and run dosbox.exe inside the DOSBox folder.

Aureal A3D Patch for D3D
A rather obscure patch, Aureal made a patch for MDK that implements A3D Audio into the Direct3D version of game. There was also a version for the Software-rendered version on Windows (MDK95), but this has yet to be found. Linked below is a version of the patch with the CD check removed. Just extract to your MDK installation and run Mdka3d3d.exe.

This isn't a patch, but if you want to get MDK95.EXE working on your modern machine, try downloading dgvoodoo, get the settings how you like it and then set the compatibility mode to either Windows 95 or 98. That should do the trick.
