I have kept a record of all items that I have received in the original game - and it keeps increasing. In my latest game I purchased two new items that I had never seen before (The Sword of Pendulum and The Master Crossbow).

Is there a list anywhere of all possible items? Or are some of these just 1-off items that are generated during the game?

So far I have completed 189 games and found 213 different items.

Based on the names and such, I'm pretty sure there is just a pre-generated list that is really long. Many of them don't show up because the game doesn't last long enough; if you play a single game long enough, you should stop seeing the weaker stuff you've already bought and broken.

It looks like there are 234 items findable in the original game (1.31), and 14 more (including the two you mentioned) purchasable from the merchant. Later bugfixes make two more items available.
The game actually includes an application which allows you to see the entire list and make adjustments to each item. Inside the game files there is a file called itemmake.exe. Run it by dragging and dropping it on dosbox. I'd increase the default number of cycles.

The itemmaker lists a total of 250 items, not all of which probably show up in the game due to bugs.
