My GOG Mafia install always showing setup instead of game - even after selecting settings parameters and clicking "Confirm and exit". What can be done about that?

Looks like you ask about Mafia_settings.exe or something like that. As i remeber it not work as a launcher.

Which one version you use - GOG or GOG Galaxy one?

Gog Galaxy

after clean install game always launches settings. After you manage them and press save the launcher launches the game itself.

So this does not happen in my case. Launcher always opens settings, like it's the first time
