Xuang9: It's easy to remember which teleporter goes where.. unless you put off playing soul reaver in which case you could replay the whole game, read a faq, or go to each porter and write down where it is that goes where.
Yeah that's that easiest way, I think many gamers today have forgotten some things that used to be common place in gaming and that was to have a note pad or something to write notes. If you look at many instructions manuals for games, especially some older ones, many have a "notes" section in the back. Back eons ago we gamers had to write down clues to help figure out stuff later on or codes to resume our progress. While I'm certainly not part of the group that says gaming today is easier, it is more streamlined and I think a lot of gamers have forgotten or some of the younger ones haven't experienced some of the old school methods of keeping track of things.

Get a pad of paper and a pencil and when you come across a new gateway draw some crude quick copy of it and where it goes then you have that reference sheet for later.

Or I'm sure you can just go to one of the LoK sites and print something that indicates where each goes.

morciu: that damn teleporter, how the fudge am I supposed to remember where are those crazy symbols lead to, they could have given them some names or something. I can't use it and I always find myself getting angry at it and going on foot to wherever i need to be.
Going a bit necro, but actually, there aren't that many of them and most of the gates have symbols that represent what is in the vicinity:
http://www.dark-chronicle.co.uk/sr1/warpgates.php
so:
Abyss - a spiral, just like the vortex in the Lake of the Dead
Raziel territory - just like the symbol on your "scarf"
human citadel - like a building
two Zephon gates - remind me of spider legs
Tomb of the Sarafan - looks like what it is
stone glyph - looks like a stone
Rahab territory - looks like an eel
sunlight glyph - looks like the sun

Also, the gates are ordered in a sequence in which you progress through the game (more or less), so it's not that difficult to keep track of. The one for the Abyss is the most used I think, as it's sort of a hub which leads to Sanctuary of the Clans, Raziel territory, Human Citadel and Dumah territory.

I actually prefer this travel system to the more recent elder scrollesque "follow the arrow instant teleport" BS.
