Hello. I'm playing SR2 on Windows 10, with a 360 controller. Has been working great so far. But I have an odd problem where the transition between Physical and Spirit realms (And the other way around) Keep stopping as soon as I start them. The only way I can make it transition and continue the game, is to keep opening and closing the Esc menu. Is there any fix for this?

I have the same issue when I switched between the realms also, he is just stuck in the same areas and seems to be stuck in a wall. Feels like GOG sold us a broken game THEY NEED TO FIX THIS
Download dxwnd (for safety reasons, I recomend from sourceforge), run it as administrator, then click edit-> add. In new window that pop-up set path to your sr2.exe location, then go to Timing tab and set frames per second limit with delay 1msec, next go to DirectX tab and set dx version hook to dx8. Click ok, and you should have soul reaver 2 with green circle on list. Right-click on it and select run. From now on, this is how you will start the game. That should fix all isues like plane change freezing, blinking combat and too sensitive camera during swimming. Alternativly, you can import predefinied setting for soul reaver 2 from exports directory that is in the same location as dxwnd.exe, but you have to set fps limit with 1msec delay - this is essentioal for this fix to work.

The same method works for Blood Omen 2 and its menu freezing.
