Nope. Once you get the Holy Grail it's impossible to get physically stuck, and it's not possible to make the game unwinnable through your actions, either.

It IS possible to miss out on some items, however. There are a couple of weapons that are behind puzzles that can only be attempted once, and there's a health upgrade that can only be retrieved before you've killed all the bosses. There's another health upgrade that can only be retrieved DURING a fight with a mini-boss, as you have to use one of his attacks to reach it. (You can fail the puzzle for the Angel Shield too, but it can be bought from a store so it's not really lost.)
There was a way to get irrevocably stuck in the WiiWare Version. In the Graveyard of the Giants, there is a false wall you have to go through to get to the grail point for the area, and you need to visit all grail points to beat the game. One of the traps in the room with the false wall, if activated, made it impossible to get to where the wall is. This was fixed in the PC release by moving the false wall to higher on the screen.
There was a long discussion about this on the official site, and the developers said that there is another way to get to the grail point even if you block off one of the paths with the falling ice.

Of course, if you miss it, you *can* apparently get permanently stuck if your wanderings take you where you shouldn't be. I still can't get out of Spring in the Sky after the door closed behind me...
I frequently got stuck before I acquired the Holy Grail. One of the first areas I entered was the Spring in the Sky, using my last weight to get in there (I had no idea that the door would seal behind me after the area transition). Then I fell deep into the Mausoleum of Giants with minimal health, and had no real hope of getting out despite finding a save point. I made it as far as beating Ellmac before I finally broke open a walkthrough to discovered that I needed the Holy Grail - an item that I had no idea existed - to leave after fighting this boss.
