I am using an Xbox 360 controller wired to my computer

Joystick issues were solved by setting the type to "fcs" and the "timed" value to 0, so that's fine (thanks for the tip, by the way!)

My issue is that, no matter how I map my keys, if I map jump or shoot to one of the face buttons, the game reads it as pressing jump AND shoot at the same time. If I map the shoot key to one of the bumpers, I can shoot without jumping, but I find the best place to place the jump button is on the A button and if I press that, Jazz will jump and shoot every single time. Any ideas on how to fix?
I know it's a couple of years later, but have you tried DOSBox Pure? It automatically maps the controls very nicely. You just need to archive the game folder into a .zip file, install RetroArch, download the DOSBox Pure core, and load the game.

SimonTesla: Just got my XBox One controller to work in Jazz 2 under Windows 10. Here's what worked for me:
1. Bring up the Start menu and type "controller" to bring up the "Set up game controllers" control panel (or [Win]+R, type "joy.cpl" and hit OK).
2. Click "Advanced"
3. Pick the controller you want to use with Jazz 2 from the dropdown and click OK.

I suspect that my Razer Nostromo (which also shows up as a controller for some reason) messed up this setting for me, so I suspect this just works for most people, without having to mess around with settings. But hopefully someone finds this useful.
