I found the book and the necklace. Looked for the skull at the Alvaron's cemetery and place of execution, but found nothing. So where can I find the skull of the dead girl?

It's at the place of execution
there are 2 graves there, one of them is hers, but you will only be able to get her skull after you've cleared citadel dungeon and talked to the leader of the paladins and followed his quest
