So, I had 3 of those mini triangles on the "water"(?) level with the frog boss. I decided to go grinding and use one of my keys to open up the door that is accessible to the left of the encounter you'd experience if you, on the teleport pad for the level, head straight south, and then take the elevator.

Upon obtaining the 4th triangle in this location, the coughing and static signifying a cutscene started and then I saw the black figure that knocked me out after the tutorial show up again. As soon as the real cutscene started, my game crash. I wish I could recreate the issue, but I can't because after I restarted the game(forced me to "abort" the game from the crash logger) it saved the checkpoint where I got the triangle.

This isn't a huge deal, but I would like to see the cutscene... Does anyone have it recorded anywhere?

My game crashed as well upon obtaining the fourth crystal in the eastern area in the same location. Yust to clarify(slight spoilers ahead): After the coughing I saw the black figure appear, then kill the Drifter in some way and then my game crashed somewhere a few seconds into the cutscene. You didn't miss much. Upon obtaining the fourth crystal in the western forest area something similar happend but this time without a crash. The cutscene in both cases just seems to be the key points of the intro-cutscene in rapid succession after which the Drifter wakes up in a puddle of blood and the game continues.

Oh, that's disappointing... I thought maybe it furthered the plot...
