I can recommend Hammer of Thyrion (or uhexen2) as a good source port, I use to get the exp pack to work with Steam ver, you just need the .pak file off the CD and a shortcut with a new command line and your done. HD resolutions and improved stability are also always welcome.

A more unknown source port but pretty smooth is the UQE Engine. The guy has made source ports for Quake 1, Quake 2, Hexen 2 and Doom 3 Original.

GOG version of Hexen II works fine with Hammer Of Thyrion. Just tried it.
And, yes, if you have a copy of the Portals expansion, you cna set it up in the Hammer program with the GOG Hexen 2 .
There is also an unnoficial patch 1.14 for those interested in playing vanilla with some little corrections. It seems to restore fullbrights in the gl mode and correct aspect ratio in widescreen resolutions between other things. It is not exactly a source port but only a patch based in the original source code. It is for Hexen II vanilla. For those running PoP there is a specific 1.14 unnoficial patch in the same site below.

I have not tested myself yet. It should be interesting to know how it works with the GOG version.

EDIT: Well. The patch is here as an option, but sadly for me it does not work very well. The music seems to dissapear and the image is not so crispy. And the models seem to be garbled/broken. But well. Here it is for testing. The game runs. Bugged but runs.
