marakinta: I've played only a few minutes so far, but there is an in-game warning that "Senua's journey is over" if you fail too many times. No indication as to how many is too many, but some reviews indicate that it appears to be a bit random. Might be related to difficulty setting. I only do easy mode, and usually fail at dodging attacks, so I'm sure to die many times over. Will let you know when I get to that point.

CarbonSeed: Yes, like Marakinta says, the game has permadeath and it isn't optional.

The indicator of how many times you need to die before to get the permadeath, and be definitely dead is a mark, like a necrosis or something like that in the body of Senua, that mark grow everytime her get killed and if the mark reach her head, everything will be over. So that way you can, barely, know how many "lifes" remain to you, but there is not a more clear indicator of that.
Kinda like how many licks does it take to get to the center of a tootsie pop?
You get a fair warning at the beginning of the game. I am playing on Hard, and so far the game is super easy, I have died mostly to one strangely designed maze than to combat that is super casual, especially if you ever played Dark Souls. I have not finished the game yet, but it seems close, and so far it's right above my elbow, so I think that the limit is pretty lenient.

The necrosis on her right hand should tell you how bad it is. It grows most of the times you die. But sometimes, it fades back.

I don't know if it really is there, but believing in it made my struggle on her journey much more intense.

It is the matter of trust. I trusted the devs it is balanced and let me finish, if I try hard enough. So I tried.

That's the interesting thing, the idea behind that, I think, is to put the player in a state of constant tension and stress. It is more immersive and makes you feel even more overwhelmed, as someone with the problems that the game tries to show.
Without spoiling too much - and I'm assuming that people clicking this thread title are already looking to spoil this particular aspect - I can point out that what the game tells you is not a lie but you will not find anyone who has had their experience ruined by it. You can google it if you want I guess, but it's intended to give you a bit of anxiety and you will ruin a late game reveal by looking it up.

I think this is good summary. I just beat the game on "automatic difficulty". Overall this is a pretty easy game in terms of difficulty. I don't think you should have any hesitation playing it for "fear of dying."
