They say that good things come in small packages. Just do it already.

Of this I could not agree more. One of my biggest pet peeves in life is when a movie is shown on television and there is terrible VO dubbing to take out the colorful metaphors. This happens even if a movie is on in the dead of night on some channels. Why not just put a post it note over some areas in art paintings as well or just rip out an unsavory chapter or two from a book if a few people find that sort of thing detestful.
If I ever created something and some one told me I'd have to alter it in order to sell it in another country I'd tell them "take it as is or PFO." There are many dollars to be made in selling items containing adult content, oddly.

Kinda reminds me of something I saw when I used to have pay TV & could watch Conan O'Brien, at one point he showed that they'd digitally added a bra to a bond girl when she wasn't actually showing anything anyway. Thats insulting AND pointless!

@TheCheese33
I can understand that. I only looked at screenshots (of the amiga version) and I almost puked (I'm 20, by the way).
I don't really think it was a factor to the ESRB creation. It was more Mortal Kombat (since it was more available (arcade) and therefore more popular). Waxworks at least had a warning on the box that said that you had to be 18 or more to buy this game.

Ironic, as most of the really grotesque imagery I can recall on the spur of the moment comes from Japan :-/
