I've uploaded a very rough sketch of what I would want, and I'm thinking it would go up on my shoulder. My questions are: how much would it hurt, and how roughly how much would it cost? The ankh would be silver and the eye-shadow black.

I can't say how much it would cost, as that depends on your location and size of the piece, though if its just one color it shouldn't be too bad. As for pain level, once again mainly depends on size, and location (of the tattoo this time, not you :)). Also it will depend on the person doing the work.

DISCLAIMER: I myself do not have a tattoo, but my wife has 6 small pieces (no sleaves or anything) in different areas (two shoulders, ankle, wrist, forearm and bicep) so I am coming from a second hand observer position.
I was about to say that you don't really need a tattoo, but Death is awesome. Go for it.
It can't hurt that much, can it? You should go to a tattoo parlor and ask about the price.

I am covered with tats but pricing is up to the shop. As far as pain..pinch yourself as hard as you can in the area you want the tat and hold it there for at least an hour. That should about mimic the pain of the gun.
Color depends on your skin..silver will probably fade fast and basically would end up flesh colored. Sun, chlorine destroy ink..

Well, if you're going to get a comic book tattoo the you could do a lot, lot worse than Sandman. Although I appreciate that you're more interested in the ankh than the origins of said ankh, the origins of the imagery will be important as you'll be stuck with it and it will be a talking point. So keep that in mind.
to get a bbetter deal, go somehwere there are a LOT of custom work based tattoo parlours - my cities full of them and they're getting competetibe with pricing. those that dont deal in custom work so much may charge more.
also, check out their work on "canvas" if you can before you go (a good google search can help here) as they may not be up to the job.
as to pain - if yer bony built, then it'll hurt more the closer to the bone - my own shoulde rpice only began to sting as it approached the nobbley bit of my shoulder blade.
