SCPM: System Shock 2 just had me on the edge of my seat the entire time. it was all the little details like enemy chatter, the dark corridors, the eerie background noises, ghosts, the creepy character animation, and disturbing audio logs. After all these years, this audio log remains my most vivid memory of that game:

Those *BLEEP*ing monkeys!

Gazoinks: Those *BLEEP*ing monkeys!
Ugh, not the monkeys! *shudder* Those psy-blasts and jump attacks always freaked me out. I hated dealing with them more than any other enemy.
The moment when I saw my dog tripping over the power cable of my console, while I was playing ! (NOOOO GOD NO .. )
Most of the horror games I like don't seem to work like this for me; i.e. I thought Amnesia and the Fatal Frame games were scary, but I don't recall individual scenes that I'd point to and say, "There, that scene!" Rather, it's the cumulative effect, a sustained atmosphere.

Having said that, I guess one scene that does spring to mind is the locker scare in the original Silent Hill.

Aaron86: If we're going to be talking about the Half-Life games, I better mention this part in Half-Life 2: Episode 2. I'm going through these abandoned buildings to reach the car, and at one point there's a poison headcrab zombie in a locked closet. It's moaning freaked me out the first time I played through this part, mostly because I heard it before I could see it and because it almost sounded like it was singing.
I remember that too !

But the most frightening part in all the HL² series was the whole Ravenholm level, I think. Especially the first time I met those poisoned-crabs-throwing horrors and when you have to wait on a roof for an elevator to come while resisting those jumping aliens-like monsters...
Dragon Age 2, Merril and Hawke in bed, scarriest shit ever.

When I hit the sex scene in DA1 for the first time, I laughed so hard I thought I was going to pass out. Bioware's gotta learn their own limitations and knock that shit off, because I refuse to believe that anybody finds it "sexy."

Yeah, those scenes in DAO were terribad and if i had to guess i'd say most people reacted exaclty as you did. They've managed to make it worse in DA2 though, that scene i mentioned above is seriously messed up, i still have nightmares about it.
wolf3d and its damn doors and gaps in the walls , just walking by and boom its soldier or a dog , always freaked me out back then specially since the game was running quite slow and pc speaker ftw :D
