Sometimes I create my character with some 0% ability and I improve it every level a bit, showing that that character worked pretty hard to achieve something (completely forgetting about everything else...) and now is an amazing doctor, for example!
That's true! I always like it if I can justify a perk by events that have happened to the character. I remember giving a young, naive character 'presence' after they had gained some wisdom and confidence travelling the wastes.

I like to do the same with skill points, assigning spare points to skills I've been using.
Holy cow, yes! The worst is when you have a party of 6, and you have that one black sheep that you just totally botched, but you're 5 hours in before it really hits you.
Perhaps RPGs attract the type of people who would demonstrate OCD tendencies when it comes to creating/building their characters.

I know that until recently I had seven or eight female city elf rogues in Dragon Age with the same name in my save directory because I'd be unhappy with their look in-game and kept going back to character creation.

Typically what I end up doing with RPGs is playing a game long enough to figure out how I want to build a charcater, then restarting with a plan from level 1 to MAX for how I want to build them, what companions or weapons I want to pick up, and so on.
Always get yourself a good character editor
For me I don't have much of a problem with character creation. But I have other issues with RPGs, mainly treasure chest. I must collect every single one of them in the entire game or I won't feel satisfied. Unfortunately, this usually takes a very long time.
My old roommate does this. I never understood why, particularly for Morrowind, where what class and stats you choose only matters in the early part of the game. If a game actually has different classes, I tend just to replay it a few times. My OCD behaviour in RPGs is having to do absolutely everything. I have to explore everywhere, even if it means a tonne of backtracking. I must do every quest. I have to explore every dialogue tree. I don't even consider not playing them this way. My mind is constantly taking note of the things I haven't explored yet so I can go back and do it later...
I do this in point-buy systems (I remade my first Arcanum character like five times), but I don't have the patience to do it in games where you randomly roll stats. I never spend much time on physical customization in games that have millions of sliders, though. Too tedious for me.

I don't do this, I love to improvize so I hate rigid (class based) RPG systems and second half of Lionheart.
Haha I thought I was alone! Had to restart Dragon Age 1 so many times 'cos my character kpet on looking different to the customisation options...

I have this but a second part of myself is also lazy so they come in conflict sometimes and I become confused. :P

What I don't enjoy is if large parts of the game can't be explored, if you want to, in one playthrough. It's not a deal breaker but it sure annoys me. I know it hampers replayability but I consider the first playthrough to be most important and therefore I want to be able to explore most of it (it doesn't need to be everything but most).
Have you guys considered savegame editors? They're available for many games and you can get around initial character flaws easily. I'd rather take that route than redo 10-20 hours of gameplay or whatever.

I usually switch the difficulty to easier and then replay the game with a more optimal character if I find the game interesting enough.

THIS. Editors have allowed me to salvage stat mistakes, appearance issues, etc. and keep going with the story! And don't even think about playing Titan Quest without TQ Vault if you hoard stuff the way I do....
