I think that Duke Nukem in particular was intentionally designed as such because it is the stereotypical hero.

Most of the time though, I think it's just sloth on the part of the designers. There are some archetypes that you'll never see as a hero, just as the fat bald guy, but most of the time it's a matter of them not wanting to bother taking the risk.

Duke was born from the "Action movie stereotype of the 90's" thing

The Ironic thing here is that right now he is 10x cooler than the current "Bald space marine" chiche of videogames today

Take any "modern" popular shooter and now replace the main character with Duke Nukem, the game automatically becomes 2-3 times better

Now now, as far as Alan Wake and Max Payne are concerned, realize that both of them were modeled after someone on the development staff. It wasn't just randomized character drawn up by the artists everyone liked... it was just a guy at the office that looked the part. :P

Uh, while the sprite was sometimes more of a dark blonde depending on the version, Steve Purcell's cover art and the close up shots never showed him as anything other than blonde, right from the very first MI...
