I have Theme Hospital installed and all i'm trying to do with the dosbox configurator is stretch the resolution of the game to 1920/1080 yet wheneer i click save settings in dosbox configurator it says no game is installed but it is, any help?

Are you running the configurator from the start menu or from the game folder? (I don't know how the configurator works, but if it's anything like the old graphics setup, it needs the game as a command-line argument - and running the exe directly from the folder doesn't give it that, running from the start menu shortcut does).
I was trying from the folder, it worked from the icon on the start menu, annoyingly, setting the resolution to 1920/1080 for Theme Hospital and saving does nothing to the resolution

Are you trying to stretch to widescreen, so everything is "wide", or have it be aspect correct with black bars on left and right of a 1440x1080 image?

If the latter, switch output to openglnb and remove any scalers.

stretch it so it fits the screen, not a fan of the black bars tbh

Then one way is to set it to a 4:3 res like 1024x768 and have it stretched using either your monitor or GPU driver settings.
