I was flying a low altitude mission in IL-2 Sturmovik when I shot down a FW-190. Only we were so low to the ground by that point and I was just the right distance away that I flew right over the explosion from him crashing into the ground which in turn ripped through my fuel lines and cause my plane to catch on fire. After a brief moment of wondering what happened and desperately trying to keep control, my own plane subsequently crashed from the damage killing my pilot.
I wasn't sure whether to be bemused by the irony of shooting myself down by shooting an enemy plane down or impressed with the game's modeling. :)

EDIT: Also thought of another one somewhat more in line with the topic:
Jedi Knight 2 for some reason the quick-get-back up-from-being-knocked-down mechanic didn't work very well/at all (a mechanic which I was used Jedi Academy) and the final boss battle, the guy kept knocking me down which took Kyle Katarn a seemingly endless amount of time to get back up from by which time the boss would kill me ... over and over again. I did eventually beat the level (and thus the game), but that slow-get-back-up animation which led to my death so often was very aggravating.
