First of all let me say I've been very impressed wih the repackaging and streamlined installation GOG has achieved with their installers. I do have a couple of minor niggles, both of which I know would be very easy to fix since I too use InnoSetup in my day job.
1. Rather than forcing the location of the Start menu folder to GOG.com\<NameOfGame>, provide that as a default but please allow the advanced user to change it to their liking. I for example would like to be able to specify Games\<NameofGame> because I already have my Start menu nicely laid out with all my games under that folder. Yes, I know I can rearrange the Start menu after installation, but it's a chore and if it do it then the uninstaller won't be able to clean the folder up.
2. Please make the installation of a desktop shortcut optional. As a general rule It's considered polite for installers to ask before dropping stuff on the desktop (or the quickstart tray, not that you do that) since those areas are rather personal to the user. Certainly it's useful to have the ability to create a desktop shortcut, but please make it optional.
As I say, these are fine points and the installers are otherwise flawless. Nevertheless, these two little things do grate on me each time I run the installers (I've bought 4 games so far). I hope you'll take these suggestions on board: the defaults would still be available but power users would gain the ability to tune things a little bit more to their liking.

you do have the option of creating a desktop icon or not.

Gosh-darn it, so I do. I sit corrected on that one, +1 to you.
That said, the option is a bit buried. Personally I still think it's better manners for the default experience to ask explicity before adding to the desktop (let alone the quickstart tray), and that's always been the feedback I've had as a developer of desktop software. On the other hand, I can see the value of a default install with minimal keystrokes. I'd be interested professionally to hear what people here think about putting a shortcut on the desktop by default.
My other request, to allow me to choose the Start folder on the options page, remains.
