It's not that complicated. It's made for gamepads from that era, which didn't have many buttons. It has actually less hotkeys than MI1 and 2.
You will probably memorize the most important hotkeys ("E" for "Examine", "I" for Inventory, "U" for "Use", "T" for "Talk"), but most of the time you can work just fine with the context sensitive options.
As an alternative, play it with gamepad, that's what the game is designed for.
Also be aware that you can change the movement direction option. Select the one that suits you better (character relative, camera relative).
When I first played it, I also had some problems in the first scene (turn guybrush, pick up coals, rotate him again, use coals with canon). But that's actually one of the worst scenes in the game, later it gets better.
ps: If you really can't live with the controls in this game, also don't buy Grim Fandango. They are almost identical.
pss: I prefer to run the game in ScummVM. That has better modern graphical options (won't change the screen resolution for example). Scaling + filtered graphics are a big plus for this game.
