Do not know that game personally, but I have seen that screenshot on a certain respectable (IMHO) Abandonware site.

EDIT: On inspecting further, it turned out that screenshots are actually different, they just looked similar, so I could be wrong after all...
You're correct, the screens are different because I took this one myself ;p

I think that's "Meat Puppet" aka "Lotus" or something like that. Played the demo back in the days and never heard of it again. IIRC it got bad reviews, but actually I've been looking for it. :D
