Vythonaut: Kerbal Space Program. It has all the elements you seek, plus many more. Some things you can do:

● Build a plane, fly it in Kerbin (Earth), try not to crash.
● Play Search & Rescue. Place some Kerbonauts somewhere and try to save them. This scenario will happen to you when you first start the game anyway. ;)
● Build a rocket. Fly to Mün (Moon). Plant a flag. Be proud!
● Build a rocket. Build a rover. Find a way to bind them together. Go to another planet. Take the rover for a ride there.
● Same as above, but instead of a rover, build a plane. Find a planet with an atmosphere. Fly your plane there and write history.

Seconded. Might be a little more sandbox-y than you are looking for, but it's a great game that does include everything you are looking for.

I've been killing Kerbonauts since 2014.
Your request is oddly specific and knocks out otherwise good canidates like FlightGear (No eject and ground) or SimplyPlanes. (No people.)

I do however have three caveats;

2) The ability to switch between ground and air should be an element of gameplay -- so games where ground missions and air missions are separate are out.
3 The game world can't be randomly generated. Nothing like No Man's Sky or Minecraft or Notcis.

I've found games that match a few of these criteria, but none with all three. The closest thing I can think of is the old Mercenary trilogy that started in 1985, and even that's a stretch. Something like Rogue Squadron except where you can organically land your ship would be ideal.

So yeah, anyone know of anything? I'd appreciate the help, this is something that's been bugging me for awhile.
Easy, the title of a game like this is Acrobatic Dog-Fight. And arcade game from the mid 80s. One of my favorite arcade games back in the day and one of the few arcade games I actually finished.

This is the second time I am posting this. For some reason my previous doesn't appear.

A game such as the one you are looking for is called Acrobatic Dog-Fight, a mid 1980s arcade game. You fly a WWII -ish plane and you try to reach the end of the game, you fight enemy planes, flying cats, balloons with witches, and of course alien spaceships. You can at will eject from your plane, to grab a nearby enemy plane in case you want to or in case your plane is damaged. You can eject and use your parachute to land and grab another proper (your army) plane. Of course you have to be careful as an enemy dragon can and will burst through the ground to try to get you before you get on your plane.
I know the game because I played it a lot in the mid 80s and it is one of the few I actually finished.

tinyE: I can't believe I remember this but Captain Goodnight had a lot of ejecting out of planes and jumping out of helicopters.

BenKii: The closest game I can think of that might meet all your criteria is Star Citizen. It's currently an early access title but promises to have all those features in the final release. You can check the current progress of the game here.

Well yeah it's oddly specific -- I already own plenty of general flight games and they aren't hard to find.

Thanks for the suggestions everyone; none of them fit perfectly but I didn't really expect them to. There's some untapped potential here that some developer really needs to take advantage of!
The Just Cause series fits all three points... though the first point only technically--main focus of the game is indeed flying, but with your parachute and hook rather than a specific vehicle. The fact you can jump from an helicopter into an enemy plane and hijack it in-flight, not as a QTE or cutscene but an actual, "miss the enemy plane and go *splat* on the ground" action should give you an idea of the freedom it provides on that matter, though.

Hmmm I'm suddenly reminded of JetFighter, which does landing/takeoffs from a carrier, and you can eject and just float down to the ground, which can take a very very long time after your plane has crashed...

I believe that F-15 Strike Eagle II did the same thing.
Do you try Parkan 1 and Parkan 2? Semilar with No Man's Sky but without procedurally generated stuff.
