They had the brilliant idea of saving the settings in a binary file instead of clear text... sigh

In Windows 10, the OPTIONS file is in Users\[USERNAME]\AppData\LocalLow\Microids\Flashback

You should check if the file gets written to when changing the options in game. The write to disk is done when you press "Back" to go to the main menu after making the changes. If the file is not being properly written or if it appears the game cannot read it when loading, for any reason, please let me know.

Just checked, it gets written indeed
If I was you, I would try uninstalling and deleting all remaining files before trying again. After that, it's contacting GOG support :(
Known issue. I can confirm the same: I don't use Bloom. After I saved the game with Bloom OFF, when I quit and restart the game, and Continue... Bloom UI = OFF but it is actually ON and you have to change it On/Off again every single time.
