Having the same problem - "Disconnected from Server" upon starting a new singleplayer game, after the loading screen.

Changing texture resolution to medium (or any other setting to low quality) does not help. Anyone having the same problem? First several hits on Google (4GB RAM patch, disabling secondary screen, etc.) did not help either.

Yep, I'm having the same issue at the end of Terminus Map, F.E.A.R extraction.

Try the Force Restart option in settings. Otherwise you'll need the 4GB patch or the Large Adress Aware tool. I'd give the link, but even if I put the link in quotation marks I'm told I'm not allowed to post links. You'll have to google.
