It's the cruelty and lack of consideration people show to a character with low intelligence, and the cutting remarks they make, which is part of what makes Fallout what it is.
I'll always remember how bitterly disappointed was the doctor who raised my PC's intelligence from 1 to 2 in the original Fallout, lamenting (briefly) that her surgery was a failure. It wasn't, of course. It doubled his Intelligence score, a more dramatic change than any prior, but doubling the IQ of a rock makes him as smart as two rocks.
