I use some mods on PC, but the game dont start by GOG the launcher and the achievements and save sincronization not work anyone know how to start the game by the NVSE and use GOG launcher?

Just rename nvse_loader.exe to launcher.exe

What worked for me was renaming the existing FalloutNVLauncher.exe to something else (for example, originallauncher.exe) so that GOG would ignore it and then rename nvse_loader.exe to FalloutNVLauncher.exe. Hope this helps.
