I haven't played this game since about '03. How is the difficulty for getting back into it?

As with all Bullfrog games, it gets pretty tough down the line, but the tutorial levels should have you covered regarding the basics.
As for graphics, well, it's an old game, it may not go easy on the eyes, especially with the default resolution -- fortunately, you can double it by pressing Ctrl+R when in-game. Then again, if you played it in 2003, you probably already know this, and I assume it's not an issue to you.

If you find the official Dungeon Keeper and Deeper Dungeons campaigns too easy, I highly recommend the Ancient Keeper campaign. Some of these puzzles are so tough, you'll probably need to get help from the walktrough : http://keeper.lubiki.pl/dk1_mpacks/ancient_keeper/ancient_keeper_walkthrough.htm
