I just bought a wide-screen monitor and my max resolution is 1920x1080, but when I start the game, it's full screen and way too pixelated. I'm trying to use the graphic mode setup, but I don't know what resolution to use and in the basic settings, I don't know what graphic mode to use. I don't know much about any of this stuff including aspect ratio and double buffering. Please give me any info you can. Thanks.
Are we talking more pixelated than it already should be, considering it is a game that was built for the age when monitors only had a maximum resolution of 640X480? This game should look fairly pixely on almost any decent-sized modern monitor, regardless of the options you choose in the launcher.

That being said, on my system it seems to look and perform its best with the graphic mode set to "DDraw", the "Keep Aspect Ratio" option checked and the scaling engine set to "hq2x", but you may prefer other options. I should mention that I had problems running the game if I chose Direct3D or OpenGL for the graphic mode.
The game runs at 320x200 (which most DOS games do), and it will look pixelated no matter what.

I guess you could try changing the scaler from normal2x to hq3x but that's really all you can do.
Thanks guys. That helped a lot. I was able to get it down from full screen so the pixelation was not so terrible. I knew this game was supposed to be pretty rough graphics-wise. It's actually the first computer game I ever owned as a little kid and this thing used to crash my parent's first computer all the time because it required so much memory. Too funny.
opengl is the best option if running in a window (with nvidia cards anyway).
Try to change the scaler. I recommand setting it to

scaler=2xsai

as I think the 2xsai scaler works best with hand painted backgrounds.
