I don't know if anyone has a fix for this but I keep having this issue where I enter the Lothering tavern and it's fine the first time when I grab Leliana but the second time I come into the tavern the game crashes. I have tried this about five times in a row and it works like clockwork.

Lothering has an issue, in the Tavern and the chantry.

lower your graphics settings and resolution and then try your last save... when you get through those 2 areas, save and then crank up your graphics and resolution settings...

That is what helped me, hope it helps you.
