GZDoom and ZDoom combo over here, use GZ for the more advanced mods while I use ZDoom to play it semi-vanilla style with some enhancements such as a high framerate, high resolution, and improved mouse controls.
Personally, when I want to play DOOM's as I remember playing them as a kid, Chocolate Doom is my go-to source port, which aims to reproduce the vanilla feeling (and that means low resolution but more faithful midi support) WAY better than dosbox'd ever hope to achieve. It also has pc speaker sound option.

Otherwise, [G]Zdoom or Vavoom.

How so? DOSBox doesn't process MIDI directly. It sends it on to the OS for playback. Install a virtual MIDI synth of your choice, I happen to use fluidsynth, and a SoundFont of your choice, Timbres of Heaven for example.
I agree. IMO it doesn't get any more authentic than playing in DOSBox. Apart from playing it on a real DOS PC that is :)

Dan113841: I stick to Doomsday Engine with the HD plugin and Abbspack.

Gzdoom if I'm feeling nostalgic and want sprites instead of quake-like polygon entities.
Now that takes me back

While Doomsday has been a staple of my 'Puters ever since PCGamer did an article about "a new Doom engine that'll spoil your retinas rotten."

But with the GOG release I'm trying out GzDoom+brutal Doom & a Visor hud mod

You could always move to one of the offshoots of Brutal Doom instead, then, like Brutal Doom Johnny Edition (BDJ) or Brutal Doom Painkiller Edition (there are others, too). These use the base Brutal Doom mod but the guys doing these offshoot mods have then applied various changes to it, like incorporating other mods, to get what they want out of Brutal Doom instead. I find BDJ to be great fun.

I generally play the original single player levels. PrBoom+ faithfully recreates the original games while offering high resolutions, uncapped framerates, an OpenGL version (GLBoom is included in the zip) an up/down mouselook in the GL version
