I recently purchased The Ultimate Doom and tried to run it. Whenever I run it on the included DosBox there are no monsters. I tried running it on PrBoom and GZDoom to no avail. No matter what I try there are no monsters on the Hurt Me Plenty difficulty. There are some on Ultra-Violence and higher but none on the difficulty I want to play on. I checked the WAD's MD5 hash and it matches the published MD5 hashes for the ultimate doom. I have no idea what is going on.
