Schwertz: Before asking for guidance describe exactly what are you doing exactly and what are you trying to achieve.
If we speak about original DosBoxed DOOMs (FInal DOOM included) purchased from GOG, they have their respective IWADs uncensored. No special actions required.
If we speak about Nightdive's DOOM+DOOM II port for Windows 10 purchased from GOG, it has all IWADs censored (Final DOOM included). Medikits with a green cross instead of a red one, etc.
I don't understand what exactly "works" for you but miracles just do not exist.

Nope. That's not the case. I happen to have this on Steam as well. And there situation is the same: in directory base reside DosBoxed uncensored versions. While in the rerelease directory lies Nightdive's port with censored IWADs. See screenshot.
So, once again, if something "just works" for you without any additional effort then you either run old DosBoxed version of the game or use prepatched distribution which could not be legally purchased one. Honestly, I'm not really interested in playing the role of psycher-detective. So, conversation ends here.

Legally purchased new Nightdive's port of classic DOOM IS censored on both Steam and GOG.
You HAVE to patch it manually to get censored contents back. Alternatively one might just take IWADs from the DosBoxed versions of the games (which are available on both Steam and GOG too) and use with 3rd-party source-port of choice.
