Party members still here but goes separate, not any more in formations, wont follow each others, is it a bug or i miss something?

Click and drag portraits together to have characters follow the selected leader (hold left trigger when using a controller, then left stick and buttons to select, split, join and reorder portraits).

i play this just few days , like it VM, reminds me of good old RPGs like BaldursG,

Party is limited at 4 members is it permanent or later in game can be more for examp depends on leadership points or so?*

That is by design. There are a couple mods that can increase the party size.
